Article 1. Purpose
1.1 Gestalt Wellness Institute Southeast Asia, Inc. ("GWI") offers free online questionnaires (the "Tools") so that adults can check, for themselves, the kind of support they may need.
1.2 The Tools are screening instruments. They are not a diagnosis, an assessment, a consultation, or treatment, and using them creates no professional relationship with GWI.
Article 2. Who may use the Tools
2.1 The Tools are for persons aged 18 and over.
2.2 A result is meant to guide the user's own next step or to be shared with the user's practitioner.
Article 3. Not for emergencies
3.1 The Tools do not monitor answers and cannot respond to a crisis.
3.2 A user in danger, or thinking of harming themselves, should call the crisis lines shown on the page or 911, or go to the nearest emergency room.
Article 4. No data collected
4.1 Answers are scored on the user's own device. They are not sent to GWI, stored, or added to the web address.
4.2 Acceptance of these Terms is not recorded.
4.3 Other parts of the website, such as cookies, analytics and chat, are covered by GWI's Privacy Policy and never receive the answers.
Article 5. Results
5.1 Scores and descriptions follow the published scoring of each instrument.
5.2 A result can be wrong or incomplete. Only a qualified professional can interpret it for an individual.
Article 6. Instruments and credits
6.1 PHQ-9 and GAD-7: developed by Drs. Robert L. Spitzer, Janet B.W. Williams, Kurt Kroenke and colleagues, with an educational grant from Pfizer Inc.

Article 7. Limitation
7.1 GWI provides the Tools with reasonable care and without any other warranty.
7.2 Nothing in these Terms limits liability that cannot be limited by law.
Article 8. Changes
8.1 GWI may update the Tools or these Terms. The version and date are shown on this page.
Article 9. Governing law and contact
9.1 These Terms are governed by the laws of the Republic of the Philippines.
