3. Your Placement

Placements are in person at GWI’s Cebu clinic. All trainee work is supervised.

A pre-admission psychological assessment to determine your fitness for the program — required of every internship and CMRP applicant — and an interview.

Advanced standing (grandfather clause)

Complete this section only if you chose Advanced standing above. It follows GWI’s Form F: a psychological assessment completed before you began practising, at least one (1) year of practice with GWI, and at least thirty (30) weeks of supervision, in good standing and licensed where applicable. No fees apply.

I certify that I completed a psychological assessment before I began practising with GWI, that I have practised with GWI for at least one year, and that I have completed at least thirty weeks of supervision, as stated. I apply for advanced standing and agree to the Chief of Professional Services’ review of my requirements, and I understand advanced standing does not exempt me from current program, scope, and client-protection policies.

4. Consents & Attestations

Each item below is separate and required.

I confirm that I am 18 years of age or older. I attest that all information and documents I have provided are true, accurate, and complete. I understand that submitting this application does not guarantee a placement, and that any placement depends on GWI’s screening, available supervision, and the applicable written agreement (for example GWI’s Clinical Mentoring and Residency Agreement, or a Memorandum of Agreement with my school). I understand that every internship and CMRP applicant must undergo a psychological assessment to determine fitness for the program as a prerequisite to admission (advanced-standing applicants instead submit proof of the assessment they completed before they began practising), and that, if admitted, I must complete GWI’s trainee onboarding and orientation before I begin. I give my free, specific, and informed consent for Gestalt Wellness Institute Southeast Asia, Inc. to collect and process my personal data and sensitive personal information (including my educational records, curriculum vitae, school endorsement, where applicable my PRC license details, and, for advanced-standing applicants, a summary, de-identified portfolio and supervision record of my prior practice with GWI and the date — not the results — of my pre-practice psychological assessment) for the purpose of evaluating my application for internship or training under the GWI Clinical Mentoring and Residency Program (CMRP), verifying my school endorsement or credentials with the issuing institution, recording whether I have completed the pre-admission requirements (a psychological assessment and trainee onboarding and orientation — completion status only; assessment results are kept in GWI’s confidential clinical records and not on this website), administering my placement if I am accepted, and, if I am shortlisted or admitted, processing my assessment, enrollment and tuition payments through GWI’s payment processor (HitPay), with the payment records kept as financial records as required by law, in accordance with the Data Privacy Act of 2012 (RA 10173) and the GWI Privacy Policy. I understand I may exercise my rights, including access and erasure, by contacting the Data Protection Officer at privacy@gestaltwellness.com.
